CLICK HERE TO DOWNLOAD AND COMPLETE THE INTAKE FORMS PRIOR TO YOUR APPOINTMENT.

You are more then welcome to complete the forms in the office; however, please arrive 15 minutes prior to your scheduled appointment so you have time to complete them.

The first session lasts approximately 90 minutes; first time appointments will entail consultation and treatment, incorporating a detailed conversation to review your health history, and to begin formulating a strategy to meet your health goals.

Follow-up visits generally last about an hour.

Preparation Guidelines include the following:

  • be sure to eat up to two hours prior to your scheduled appointment, though not too heavy a meal

  • if your normal practice is to brush your tongue, refrain from doing so, as examination of the tongue and tongue coat are part of the diagnostic process

  • dress in loose, comfortable clothing

Let me know if you have specific questions or concerns.

IS THERE ANYTHING I SHOULD DO BEFORE MY FIRST VISIT?

Please bring any information about your case that you may have received from your other doctors such as X-rays, lab test, blood work, MRI's, etc. As with any medical visit, it's a good idea to write down questions you may have ahead of time and keep a running list so that you don't forget anything you'd like to address or ask. I'm an advocate of patient education, therefore no question is too inconsequential or off the table.

Try to arrive 5 or 10 minutes before your appointment to give yourself a chance to relax. Comfortable, loose clothing should be worn that can be pulled above the knees and elbows. Try to avoid the following:

  • Eating a large meal within one hour or your appointment

  • Fasting for more than six hours before your appointment

  • Alcohol, tobacco, food or drinks that color your tongue (such as coffee) immediately prior to treatment

  • Vigourous exercise or sexual activity within the hour before treatment

CANCELLATION POLICY

In fairness to both new and returning patients, the clinic maintains a 24-hour cancellation policy. You will be required to provide a credit card number (including 3 digit CVV code) when scheduling; this will ONLY be used if canceling within 24-hours, in which case you will be charged for a full treatment.

INSURANCE

I accept all insurance that covers out-of-network Acupuncture treatments. We can work together to confirm the extent of your coverage with your carrier.
